Your Perfect Select Rewards is redeemable for a reward and/or reward certificate as outlined in your brochure. Pictures of products on reward brochure and reward certificates are for illustrative purposes only and may be subject to change. Perfect Select Rewards will fulfill in accordance with the terms and conditions of actual certificate selected. For participant to receive reward, they may be subject to deposits, travel fees (if applicable), shipping and handling charges and/or instructions, self addressed stamped envelope for return postage, copy of Government issued ID and/or proof of purchase - please see actual reward selected for terms and conditions that apply to your reward. Terms can be viewed online at perfectselectrewards.com when registering your reward. All coorespondence must be returned in the designated time frames, and by certified mail if applicable, or offer is null and void. Perfect Select Rewards is not responsible for mail that is lost, mutilated, or delayed. Any reissue of certificates will be assessed a fee of $25. The information on the Perfect Select Rewards Brochure is provided for informational purposes only and does not constitute an agreement and/or endorsement of any of the the products or certificates mentioned. Products and/or certificates mentioned have been selected by your sponsor as a Thank You. Perfect Select Rewards agents, assignees, and or delegates act independently of the sponsoring company who issued you the Perfect Select Rewards as a thank you. The General Information Provisions under which you agreed to cannot be changed or amended by any person or company without the written consent of an authorized officer of Perfect Select Rewards. See actual certificate and acknowledgment form for complete terms and conditions. If product or certificate becomes unavailable to Perfect Select Rewards we have the option to substitute another reward incentive of equal or greater value. No recipient should rely upon representations other than those included in the actual terms and conditions of the certificate selected. Fraudulent submission could result in federal prosecution under the U.S. Mail Fraud Statues (18 U.S. Code Section 1341 and 1342.)
